Dialysis centers have a drain problem that almost no other healthcare setting shares. Every treatment station discharges used dialysate — the spent buffer solution pulled from a patient's blood — down a wall box drain or floor sink. That waste stream carries two things that destroy drain lines: bicarbonate concentrate, which deposits calcium carbonate scale on the inside of the pipe until it narrows and clogs, and organic metabolic waste, which coats the drain walls with a rich biofilm.

The CDC has documented that wall box drains in dialysis centers are 'predisposed to the development of biofilms' and that those biofilms can harbor opportunistic pathogens tied to healthcare-associated infections. That same biofilm is what breeds drain flies (Psychodidae) — the small dark flies that appear around the floor sinks and travel into the treatment bay, where patients notice them immediately.

Why standard janitorial programs miss it

A mop and a quat disinfectant clean the floor around the drain. They do nothing to the scale ring building up inside the pipe or to the biofilm coating the wall of the drain line ten inches down. Acid drain cleaners strip the mineral but destroy the biological ecosystem the drain needs to stay clear — and the scale grows right back in weeks. Pesticide sprays for drain flies knock down the adults but leave the larval mat intact in the biofilm, so the population rebounds within a shift or two.
